Transcript Request Instructions:

There are 2 options to request transcripts from the Waipahu Campus (includes Kapolei and Waianae sites).

Option 1:

To pay with a credit card, debit card, or Amazon Pay, click here: Transcript Request Link

Option 2:

To pay by cash or money order:

Print and fill out all fields on the Waipahu Campus Transcript Request Form below. PLEASE PRINT LEGIBLY!
Attach a clear copy of your State I.D. or Driver’s License.
Payments of transcripts are accepted by: Cash (exact amount) OR Money Order (payable to D.O.E. – State of Hawaii)
Mail the completed form, copy of your I.D., and payment to the following address:
