Fill out the Wahiawa Campus Transcript Request Form below, print, and submit it with the required documents and payment
Online Payment Option: Please do not make any online payment prior to submitting the Transcript Request Form and receiving verification from our office.
Click here to pay for transcript online.* After creating account/logging in, select Wahiawa Campus, then click on Purchase Supplies.
